The VNA of Mercer County is a leader in disease management and in achieving improved patient outcomes with over 70 years of home care experience. Through the added use of Philips web-based clinical review technology, the Agency provides at no cost to the patient or their insurance company, remote monitoring for patients with chronic conditions, such as (CHF) Congestive Heart Failure, (COPD) Chronic Obstructive Pulmonary Disease and Diabetes Management.
Through partnering efforts with the patient in the comfort of their home a Nurse Coordinator at the office is able to quickly assess the status of a patient from their home and arrange intervention and follow-up for those most in need. This provides "real-time" objective data for the physician and allows more timely intervention which focuses on reducing unnecessary emergent/urgent care; avoiding costly re-hospitalizations and improving the patients' quality of life.
The use of this revised technology supports and enhances the services of the Agency and is an effective tool in helping providers manage patients with chronic conditions.
Key elements of the program include but are not limited to the following: remote access technology; graphical chart trending; patient validation surveys (triggered and non-triggered); real-time updates on weight, pulse ox, blood pressure and patient glucose findings.
In addition, the Agency has ZOE monitoring capability, which measures early changes in the patient's condition up to two weeks before dehydration or CHF is identified through symptoms expressed by the patient or found by the clinician.
