VNA nurses, therapists and home health aides incorporate our falls risk identification and prevention measures for all patients receiving our care. Falls are the number one cause of injury-related deaths in older adults. Our staff provide in-home safety assessments and make recommendations regarding how to improve safety in the home environment. Physical and occupational therapists provide rehabilitation services and education to address falls risks from balance, coordination and strength impairments. They also instruct patients and caregivers in safer ways to walk, transfer, and perform activities of daily living or household tasks.
