"COACH" to Prevent Falls. Community Outreach to Assess and Correct Hazards

COACH to Prevent Falls is a multi-phase education, outreach, and assessment program developed by the Visiting Nurse Association of Mercer County to reduce falls in the home environment. The program helps seniors identify and correct falls hazards by increasing their knowledge and promoting behavioral changes.

The COACH program addresses a major public health, social, and economic concern for seniors. In the U.S., almost one-third of adults aged 65 and older fall each year. Of those who fall, two-thirds will fall again within six months. Adults who fall are often afraid to continue an active lifestyle, but as they walk and exercise less, they become weaker and less agile and more likely to fall again. Those who fall are often unable to regain their ability to care for themselves. Forty percent of nursing home admissions are related to falls. Research shows that many falls can be prevented with risk identification and correction interventions.
